In 1984, William Milam founded Angionet, Inc. in California. Prior to that, he had a successful career in ophthalmology that began in 1972 at the Bascom Palmer Eye Institute where he worked alongside some of the most esteemed ophthalmic surgeons of his time. In 1978, Milam was appointed as the Head of Photography at the University of California, Doheny Eye Institute located in Los Angeles, California. Later on, he served as an Associate Professor at the University of Arizona, Department of Ophthalmology.

During his time at Bascom Palmer Eye Institute in the early 1970s, William Milam faced a major challenge. The institute's photography department had four Zeiss ophthalmic cameras, each with a different film back that was incompatible with the others. When a film back broke, the camera became unusable, causing delays in diagnosing patients and putting a strain on the technicians. Additionally, the camera backs were not suitable for other types of imaging.

Fortunately, the Department of Ophthalmology had a state-of-the-art machine shop for developing surgical devices such as the VISC. Milam enlisted the help of the head engineer, Willie Aumeyer, to adapt and replace all of the camera backs with Nikon. The Nikon cameras were cross-compatible and could be used for external photography as well, solving the problem.

In early 1999, Angionet sought the expertise of Robert Hodson at Trioptron, Inc. to help convert their film-based Kowa cameras to newly released Nikon D1 digital camera backs. Given Milam's prior success in a similar endeavor, the conversion process was familiar and successful.

By the year 2000, Angionet had completed the transition of all diagnostic testing to digital format. Additionally, they designed and launched a secure website that allowed physicians to access all of their patients' images and data regardless of location.

From the outset, Milam recognized the immense value that the internet and digital media provided for creating and displaying visual content. As such, Angionet has consistently pursued the development of web-based platforms for this purpose, recognizing the importance of staying current with technological advancements in order to provide the best possible service to their clients.
